If you are a new patient to our office, please visit our Patient Forms page to download our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival. We make every attempt possible to provide you with an accurate estimation of the costs involved with each visit. However, please be advised that estimates are provided before service is rendered and insurance has paid.
Payment for services is due at the time service is provided. Patients may pay by cash, check, or credit card (Visa, MasterCard, or Discover). We also do accept Care Credit.
For your convenience, we also do accept Care Credit.
As a courtesy to you, our administrative staff is happy to process dental insurance claims on your behalf. We will make every effort to assist you in maximizing your insurance reimbursement for covered procedures. There are a limitless number of dental insurance carriers as well as group plans that are on the market. It is important to us that you understand that the contract regarding your dental benefit package is between you and your employer or insurance company. Our office is not a party to that contract and is ultimately not responsible for how you insurance handles your claims or for the benefits they provide. Because of this fact, we cannot guarantee that your insurance company will pay for the treatments you receive from our practice.
